The Care Team are expanding their management structure due to the recent HSE contract win. The role is community based in Dublin and requires a highly organised, flexible self-starter with excellent time management skills and good initiative. This is a hands-on management role based in the community. The successful applicant will be capable of managing all care aspects of day to day operations, be responsible and accountable, possess excellent communication skills and confident to lead the implementation all quality care initiatives. Role: • Case management – identifying needs and. requirements for services users, providing. assistance in achieving goals for service. users to remain in the community. • Setting up care packages for both HSE and. private clients. • Managing a team of supervisors and. healthcare assistants in the community. • Partake in MDT meetings both in the. hospitals and community settings. Responsibilities: • Introducing Healthcare Assistants to. service users, shadowing care shifts to. ensure the quality of care and up skilling. them in clinical. areas. • Report medical needs for each client in line. with the care plan including medication. management, wound care and any other nursing duties. • Participate in the planning, assessment, implementation and evaluation of nursing. care. for clients in their home environment. • Liaise with the clinical director, client. services manager, other medical. professionals, healthcare assistants and family to ensure. optimum care for the client in their home. • Maintaining medical records and providing. accurate and timely reports of any changes. in the condition of the patient. • Use evidence based practice to develop. and maintain high quality client care and. ensure the continuous improvement of. home care package. • Shared on call elements during the. weekends. Requirements: • Bachelor Degree in Nursing or Social Care. or in any other healthcare discipline. • Minimum of 2 years of Case Management. experience in the community. • Good IT skills in Microsoft office, Excel and. Outlook. • Excellent communication skills. • Current People Moving and Handling. certification. • Current CPR certification. • Working experience of Patient. Assessments and Care planning. • Full Clean Irish Driving Licence.
